Phase I Survey, Phase II Inspection,
and Phase III Annual Inspection (continued)

  1. Instruct the designated plant personnel to place danger tag and remove devices from service if they have any of the following conditions:
    1. Deformation—Hooks: Any visibly apparent bend or twist from the plane of the unbent hook.
    2. Throat Opening—Hooks: Any distortion causing an increase in throat opening of 5% not to exceed 1/4” (or as recommended by the manufacturer).
    3. Wear—any wear exceeding 10% (or as recommended by the manufacturer)
      of the original section dimension
    4. Inability to lock—any self-locking hook that does not lock
    5. Inoperative latch—any latch that does not close the hook’s throat
  2. Based on results of visual inspection:
    1. Perform additional nondestructive tests, as a minimum, to include proof test
      and magnetic particle testing designated devices and hardness as required.
      The testing may include bringing our portable proof tester on site.
    2. Design, modify, and/or replace devices not meeting your guidelines.
  3. Based on findings of Phase I, Mazzella’s may send up to four people to assist with
    Phase II implementation.
  4. Output of Phase II is to generate a documented report indicating condition of
    existing devices.
